BIRTH CERTIFICATES
After delivery, parents are given an Informational Sheet for Completion of Certification of Live Birth. The form is to be completed before leaving the hospital and given to the nursing staff in the OB Ward. Do not leave the hospital without turning in the informational sheet as it will delay the processing of the Certified Birth Certificate provided by Public Health.

INFORMATION FLOW SHEET
The Information Flow Sheet is a required document when submitting application for baby’s social security number. The Flow Sheet is available at Medical Records. Please be advised of the following:
- The Flow Sheet cannot be sent via email
- It does not require Authorization request form
- We cannot process the request until DPHSS Official Birth Certificate is provided to GMHA Patient Registration so baby’s name can be updated in the system.
IMPORTANT INFORMATION REGARDING A DEATH CERTIFICATE:
If the deceased died at GMH, please provide necessary information to our Medical Records Department of GMH located near the Gift Shop on the first floor. They will prepare the necessary documents needed which will be sent to the funeral home chosen. The official Death Certificate will be issued by Vital Statistics Division at the Department of Public Health in conjunction with the funeral home providing the services.

If the deceased passed away outside of GMH, information must be provided to the Medical Examiner’s Office located behind GMH past the Emergency Department. Their telephone number is 647-2369. The official Death Certificate will be issued by Vital Statistics Division at the Department of Public Health in conjunction with the funeral home providing the services.
